How do I generate auto e-mails based on a selection in a custom form?

Hello. I've created a report to show the # of proof that need to be validated.  That field is being pulled from a custom form.  How do I get the details and matrix (13-month rolling) to match up?  The numbers are different on the matrix (wrong here) than the details page (right #s). Thanks.Tracy
