If you have multiple approvers on a proof, it'll be pending until all approvers have made a decision. Approved with Changes counts as a decision, so perhaps you have remaining approvers that haven't made a decision.

Can you specify? A project itself isn't associated with a job role, so are you looking to see all roles associated with tasks in projects grouped by role then program?

With the new form designer, I think you're now able to copy fields with a form? You could copy a field that already has the display logic and then change field name... I haven't tried this feature yet, not sure if it copies logic.

If you're sure all things in WF are correct, have the user check their BROWSER settings they use WF in for language/locale.
