MarcusKy, The Proof API doesn't expose a lot of fields so there isn't a way to capture dates when things like changing the approver occur.  The only date fields available in API are proofCreationDate and decisionDate

Jason, the report above is a Document Version report type so there isn't a way to get to the workflow template used in the approval process.
