In my experience, people tinkering with settings they don't understand would be the only culprit. I have never seen project settings change by themselves. Are they adding a second subtemplate to any of these projects? This shouldn't change that particular setting but it's worth looking into.

Mapping a comment author to a WF account is probably way too difficult given the amount of scenarios and exceptions it would require, but a simple list of "imported" comments would go a long way. This is bothersome in the "last viewed" fields on objects as well. 

The Notifications area is only for emails. AFIK, there are no controls for in-app notifications.As you can see in the attached screenshots, I have all notifications turned off but when assigned 3 tasks and an issue, I still get in-app notifications.

Oh man, I LOVE this idea. I'd love to be able to see colours for custom task statuses in Balancer. Balancer isn't even about the project as much as it is about the task (or issue).

This is probably because you're using an attribute from a collection on a project (projects have tasks, tasks can have a milestone). Notes don't have milestone or task collections. Are you wanting to display notes on tasks that have certain milestones?
