Calculated Field on Custom Form

I'm trying to create a calculated field off of a "requested live date" calendar drop-down. I want to Display a "reason for rush" field IF the date is less than 2 weeks. Can anyone help? There are two elements at play here - Calculated field and display logic. A calculated field will only display after the request/issue/form is saved. So it won't dynamically say if it's under two weeks and do things while you're filling in the form. Display logic can only work of drop-down, radio button or check boxes that precede it in the form. So, what you might want is a mandatory drop down for 'required delivery' that has two options 'Rush Job - less than two weeks' or 'Standard SLA - 2+ weeks'. Then a 'Requested Live Date' field that has two logic conditions - if either choice is made on 'Required Delivery' and a 'Reason for Rush' mandatory field that only displays if 'Rush Job' is selected.
