Any idea what is causing the window in the Custom Forms dialog to be messed up? Is this a Chrome issue?

I have experienced screen draw glitches like this in the past. usually, it works its self out. If you try other browsers and don't see the problem try a "refresh cache and hard reload". If that doesn't work, try disabling all plugins. (Adobe PDF plugin has caused problems in the past). If you have a PC and Mac, try that.
If you can narrow it down where it's repeatable, then it's fixable.
