Issues Template - Multiple custom forms when creating an issue (requests or project screen)

Description -Ability to create a template for issues object. Ability to create a template where I can add multiple issue custom forms, that can be used when submitting a request or creating a request from the queue itself at project level.

Why is this feature important to you - I need to control visibility of certain areas only for process managers, however I want to avoid them adding a form every time they have to work on a request. Having a template would create a request with the custom form the user would complete, and custom form that the process manager would complete and would be only visible to the process manager.

How would you like the feature to work -
