Is it possible to have a master page (or another page) accept user input (First Name, Last Name, etc) and have that information show up on every page of a 5 page form? (Maybe at the top of the form near the title)
Yes it is! However you will need to select the object on the Master Page and go to the Object > Binding palette and set the binding to Global. This way the data that is typed into the Master Page object on any of the 5 pages, which be reflected on all pages.
