How do I create/edit this Send Form dialog?

Hello!

I have to edit an existing PDF form and also create a similar one from scratch - both will be sent to clients and sent back with their info.

I've just learned how to create a very simple form and it works fine, but I need to learn how to create the Send Form dialog, complete with the returning e-mail address (please see screen shot below).

This form I'm suppposed to edit already has this feature working - I just can't find where to edit its e-mail information...

I found a 67 lines JavaScript linked to the button click event and edited the mailto info, believing this would change the To: info in the Send Form dialog. But it didn't.

Please, could anyone help?
