I have a form I am trying to build that I require to be name with a unique title each time it is filled out. The title would come from within the form itself (selected from the same two fields each time).  Is this possible?

I need to find out how I can change this default background color.  Many documents that I try to open now I can't read at all.  The page appears to be all black with no writing.  This is becoming very frustrating especially when I receive so many documents in PDF format.Why would they do that?!!!
