Adding a form to a PDF document

Good Day All;

This may sound a bit odd, but here goes.

I have created a form that requires a fair amount of documentation to complete.

I was trying to figure out if it was possible to create a PDF document and as part of the PDF document there is a PDF form. When the user completes the form, they would click on an email button on the form and only the form would be emailed.

You can either attach your form to a PDF or a PDF-Portfolio, or you can design a form which hides all unwanted elements (subforms, fields...) when you submit the form.

For the last one you can use this technique from John Brinkman.
