I have not been able to reproduce this, but maybe I have a suggestion. Do you have an incomplete action? The button to add an action is grayed out if you don't finish the action you started. that's the only way I've been able to make the form stop letting me add actions. I didn't get the form that full, so maybe there's a limit on the number of actions.
