Can I add logic to a field in a custom form based on Queue Topics choice? Or do I have to create a custom form for each Queue Topics?

You can add a specific custom form to a specific queue topic, or you can add it to the queue in general. I'm not sure if this answers your question though.

You have to create a seperate custom form for each queue topic. I had same issue with one of my team. I created custom form and connect to each queue topic. Thanks
