Adding manual entry columns to a report

I'm wondering if I can add a column to a report that a user can enter a date and one that a user can enter a Yes or No answer.  Is that possible to do in a report?  I've tried looking in the documentation but I can't find anything.

hi, all columns in a report need to tie back to a field on your object. If it doesn’t already exist, it has to be created and placed on a custom form for that object. Have you had a chat with your system administrator about this need?

Thank you.  That answered my question.  I will go back to my administrator to work on updating the form first before creating the report.
