Currently, when you submit a request that uses custom forms in the request queue, the custom form resides on the Issue ('request') because that is how Request Queues are built. When you convert the Issue (again, the 'request') into a Project, the custom form data then lives on an issue within that Project. This really buries the information. It would be great if when converting you could have the option to put that data into a replicated custom form which resides on the project itself (or program - but that's another idea entirely).

Hello Mike.

When you convert an Issue to a Project/Task, if you have the same fields on the Project/Task form, that you have on the Issue, they will copy over when you convert.

We currently do this with our Project Initiation Requests. Someone completes an Issue that has a form attached to collect all of the fields that we need to setup the project. Our project custom form contains a section that has all of the same fields that are on the Issue. When we convert the Issue to the Project, the fields are copied over into the respective fields in the project custom form. Hope this helps.
