Can you expand a bit on your question?When you say users from a custom form, what specifically do you mean?  If you mean that the user would be provided a field in which they could type any name in the world - I don't think that would work - I do not think Proof HQ will have access to data in custom...

If you have access to the ProofHQ backend - have you tried adjusting the default proof preferences for the specific users you're talking about? Default Proof Settings - You can try adjusting the default proof role Default email alert settings

After reading your post I am curious if you have everyone in your instance set up with the ability to edit custom forms.  Is that site wide?  How many users have this ability?  Does anyone ever make unauthorized edits or accidentally delete important things?

I think that there is a proofing role called Moderator which is intended to fill the role you're describing.  There might be a way to have the artist maintain authorship and have a coordinator type position fill in as Moderator and as such they can route the proofs.

Once you have a custom form applied to projects it is possible to create a new view which includes custom forms as viewable columns of information.When viewing portfolios you can choose a different view, one that you build.

The basic idea to start off with might be:  Artist uploads document -> generates proof -> applies pre-set proofing workflow to proof -> modify if necessary -> share
