If we can attach a custom form to each issue as we can do today, I want to be able to attach a custom form to each document uploaded so the user can fill up additional information for the document. Hopefully, this can be turned on/off as needed per project or issue.

While I see the logic of not having a custom form from a request to a project to a task, this would only work if your custom form is meant to be the same in all 3 objects.
