Hi Julie, We have not. We use a spreadsheet and place it in a SharePoint library. Then in the WF Project (or Task) we provide the URL to the spreadsheet. But it should be pretty easy to create a form (I'd probably use a Project form). It'll just stink to have to create all those repetitive lines.

HI Mavis, Definitely yes on #1. When you create the field click on the settings (gear) on the selection and Select By Default. See attached. For #2 it gets more difficult. I haven't done it, but I believe it's possible if you use calculated fields in the form (but then they won't be editable).
