Hi Ashleigh, if you have a Fusion account associated with your Workfront instance, this is something that can be easily implemented (adding specific custom form to a document when added to a specific place or when proof is created) 

Hi All, If you're using Fusion you can create a "proof delegation" based on standard Work delegation.This is something I just put in place for one of my clients. Working perfectly!
