radiobutton groups

Hi Experts,

                  I have a table in which there are 5 columns - 2 of them are for ADD and REMOVE column. There are 4 rows under these column headers and ADD and REMOVE columns have radiobuttons. Each of the rows of radiobuttons for ADD & REMOVE columns are to be mutually linked in one radiobutton group i.e. in one row only ADD or REMOVE radiobutton can be selected.

Please let me know how can I do this with a sample pdf.

You cannot group radio buttons available in multiple cells.
