I'm trying to design a form and it contains some fields, the idea is whenever a user fill the form QR code will be automatically updated/ form data will be embded into QR code. Is it possible? If possible please guide me.

Yes, it is possible. See the Paper Forms Barcode object in the Barcodes library:
However, please note that if the user has Reader, then for the barcode to work the form would first have to be Reader Enabled using LC Reader Extensions. Reader Enabling the form using Acrobat will not give sufficient privileges. See a summary here: http://assure.ly/etkFNU.

Thank you for the quick response. I'm near to my task completion with your great help! One more thing I want to know that I also want to embed URL in the QR Code with the fields name(s). Once the user entered the form fields it have to embed into the QR Code like this one: http://www.sebcolaundry.com/services.php?name=randy&buildingname=ABC&street=hoboken&city=abad&state=...

It is possible to create a custom URL based on the user's inputs, but the issue is that the QR code will not be updated if the user has Reader and the form is either not Reader Enabled or only Reader Enabled in Acrobat.

Sir,
Form is reader enabled, I have tested it with Adobe Reader on multiple computers and it is updating QR Code whenever any field updates. I'm having the only issue is that how to embed URL int the QR Code, as I mentioned above.
