LiveCycle Form rows merge when reopening file

Hi,

I have a HAZOP form that has a 1st level row with add and subtract buttons. It also has 2nd level row within the 1st level row with add and subtract buttons.

Everything works as it should except when it comes to reopening the file after saving.

For some reason, whenever I add another 1st level row and save then open the file, that first level row is put into the 2nd level row and merging the first column. See images below. I have also attached the file here.

I would appreciate if anyone could have a look at this issue as it baffles me.
