Hi, I too am a struggling forms designer. In order for anyone to be able to see your form and help you, you would have to go to Acrobat.com which you will need to sign in to, upload your form, select "Share" from the menu, and select to "publish" the form, and then click to copy the url. Then come back here and publish that url link here, and someone will be able to help you out. It would help too if they knew what you wanted an opinion on, layout, scripting, buttons, what?
If you have trouble at Acrobat.com, look at Niall O'Donovan's screen prints under the post "Problems with a table that user can add rows to". Niall gives step by step instructions for publlishing your form.
