Extendable form - Not opening pdf file correctly via Interactive Live cycle option

Greeting!

I am creating an extendable pdf form I have an A3 form created by Adobe Illustrator but when I open form on Adobe LiveCycle via Create Interactive form it is not open correctly and I get this (Snapshot attached)

I need to open form via creat Interactive form due to table elements placement on form and create an extendable table, can anybody help me what is wrong and how to open form correctly with the original shape?
