data connection: sample XML data

Hello!
I already post a similar item before, but there was no answer, I said that I did not quite explain the problem.
I have a form, in the openning the data in an XML document must be added (name, address, postal code, phone number)
How should I do?
I tried the option data connection, it does not work, the information does not appear in the openning of the form.

Can you please check the Binding of those field is it no binding or use Name ?
