Comb Fields - with specific type of comb field look

I am attempting to make the comb fields as shown in the attached screen capture.  Any thoughts or ideas would be welcome.

You'll need to use lines. The comb feature is limited to whole boxes and not the type you have indicated. Ensure that your text spacing matches your comb. As far as I know, that's the only way to make combs look like that.

What would I need to do to ensure that the text spacing matches the comb. Are you refering to some type of kerning or spacing?  Fixed font vs Proportional fonts?
