Tutorial for: Form --> PDF --> Inbox

I am new to the forum and I hope this is the right place for this question. I apologize in advance. I am a webmaster for an association and I have been asked to create an application that we will have on our site as a "go green" project.



My goal is to have the application filled out by the individuals then once they hit submit the server will generate a PDF and send it to our inbox. I want to eliminate the individual having to send the PDF as an attachemtn via their e-mail account because it is one more step that they do not want to do.



Is there a tutorial for this, or what keyword should I type into google, etc.

What is the application that they will fill out (HTML pages)?

Here is the application that we would like to have created into a dynamic form where the individual will fill out the form and the data will be populated into the form and the final pdf will be sent to us.

What needs to be done to have this form sent to our email account as a PDF via HTTP server and not through the clients e-mail software (outlook, thunderbird, yahoo, etc)?

Here is a new form that I personally would like to know how to have this automatically sent to my e-mail after client fills this out?



I guess I have to create a php dump file that connects to mySQL and then populates the PDF, etc. How ????
