AEM forms add-on in cloud service requires a separate license. Please reach out to your Adobe account manager
AEM Forms as a Cloud Service is a premium add-on to Adobe Experience Manager as a Cloud Service. It’s different from the on-prem or AMS-based AEM Forms (JEE/OSGi).
There are two key flavors:
AEM Forms OSGi (Cloud Service-compatible): Focuses on adaptive forms, communications, and workflows.
AEM Forms JEE: Not available in AEM as a Cloud Service.
Below are some of the useful links

Hi Jagadeesh,
Thanks for your response.
I wanted to know where to get the cloud one.
Use case : We have some XFA based form for filling forms, these needs to integrated so that we fill form and data get inserted in the XFA form and there are also submission based button in the form so that we are also able to handled from outside the form and added in the XFA form
Where to get this Add on subscription ?
which onw will provide API to interact with forms ?
AEM Forms OSGi (Cloud Service-compatible): Focuses on adaptive forms, communications, and workflows.
AEM Forms JEE: Not available in AEM as a Cloud Service.

You need to contact your sales person from Adobe to get access to AEM forms as cloud service. It cannot be done without Adobe sales person being involved

@pratikba , Which organisation are you part of? Is your Org an existing Adobe customer? You cannot simply have a trial version of AEM Forms Cloud Service product.
