AEM 6.4 static adaptive form template error with the form page

Hello Team,

I am creating a static adaptive form template in AEM 6.4 for the aem forms. I can see below error when i create a form page using the base-af template.

Any reason why i am getting this error also i have seen extra parsys is getting added to the page. How can i remove this extra parsys and resolve the error. I have seen this /libs/settings/wcm/designs/default/jcr:content/aftemplatedpage  path is not there in my local crxde. 

I want only root panel for adding OOTB form components.

You can use OOTB template editor and see if you still get any error.

I don't see above errors with the editable templates but with the static adaptive form template i can see that error.
