After I click the  download button I get a new page with the same screen image and no form to fill out. The new screen image when clicked is unresponsive,

Have you tried using a different browser and/or clearing your cache?

Great!!! I copied the URL from the unresponsive page in Firefox and pasted it to the address line in Explorer and the form

came up. I filled it out and I was able to view the reports and watch the presentations. Thanks.

Not sure why Firefox did not display the form.
