Adobe Analytics really isn't a good tool for such purposes.. Adobe is about collecting behaviour and patterns, and analyzing data with a time frame.
Since Adobe is subject to following "no not track" directives, as well as being blocked by a lot of ad block/privacy blocking add-ons, you will potentially lose responses.
Not only that, you will have to use up your limited number of dimensions for each written answer; or try to combine these into something like one of your 3 available list items, or using Products list so that you aren't limited by your 255 character limit, would still not be the easiest to parse and understand your survey.
You would be better off using an actual survey software to collect survey data... however, you could create tracking on the survey to understand falloff within your survey (to understand how many people start and complete / or leave the survey and where they stop)
