Form Initiation vs Pageview

For one page (asset gating) why would Pageview be double of form initiation e12 when both are fired on page load for that one page everytime?

Please check if you are firing a page view request may be for a tab switch or click on a particular link on asset gating page. Also, see if your Form Initiation (e12) has been serialized or configured to fire once per user / visit / form submission etc.
