Metrics Reporting on a Multi Select Custom Field

I have a custom form attached to a task in my project. On the custom form is 4 sections and each section has at least 10 questions with possible answers (NA, Yes, No, Corrected).

Is there any other solution to me reporting on the questions with the answer "corrected" - OTHER than creating a calculated field for EACH of these 40 questions and then reporting that way?

Also I created one calculated field and it showed up on the report. When I added the 2nd calculated field - that one did not show up - because it is attached to the same "task" in the project.

How can I show in a report that two questions were answered as "corrected", in this example?
