Is there a way to create a report based on a custom form field?

i.e. if I've attached a custom form to a project, can I then create a report given the choices from the custom form?

You can create columns that are the fields of the custom form (see attached). Depending on how long/big your form is, that may become really cumbersome to look at.

Create a project report and search for the names of your fields from your Custom Form to be added as New Columns in it.
