I invite you to try our Update Category Generator solution, which gives you the current definition of an existing custom form as an Excel file (in the proper format you can then also easily manipulate and upload using our Update Category solution).

I know this is an old post, but maybe this will be helpful for others who have the same question.

The "Custom Data" Kick-Starts export file contains the formulas for the calculated fields in the "setCustomExpression" column on the "CTGYPA Category Parameter" sheet (tab). The value in the "setParameterID" column is the reference to the field listed on the "PARAM Parameter" sheet (tab).
