Could you share any templates you used for capturing governance documentation (change requests, issue logs, request cues, etc.)?

We don't have a template, but we house all our documentation in one project, each document has a corresponding task assigned to whoever should update that document and once that document has been reviewed/updated, we generally just push the task due date out another year rather than creating a new task.

We do use both document folders and parent tasks to group the documents/tasks either by role or by group responsible depending on the document.

I'm sure other's have stricter requirements for their documentation.
