When an issue is added to a project, an update should be made to the Updates tab so folks know there are some issues reported. The Issues label or number of issues should be in Red so they stand out. Users are not noticing the issues.
You can add an assignment on a request in a project, perhaps if someone adds a request they need to assign it to the PM or appropriate person to notify them...you can include Assignments column in your view or your request custom form to ensure someone is assigned?
