Pop-up box for comments for when a project is cancelled

We have been asked to to create a pop-up or a link to a comments box so that someone can provide comments/detail as to why a piece of work or project has been cancelled, potentially during an approval process.

As far as I'm aware, this functionality doesn't currently exist in Workfront. Would it be possible to add?

Our matter of practice is typing in a project update prior to making it Dead, if necessary.  The purpose of this would be to ensure the comment would live in a centralized location and be available to view by anyone in the future potentially looking to resurrect it.
