Pop-up box for comments for when a project is cancelled

We have been asked to to create a pop-up or a link to a comments box so that someone can provide comments/detail as to why a piece of work or project has been cancelled, potentially during an approval process.

As far as I'm aware, this functionality doesn't currently exist in Workfront. Would it be possible to add?
