Okay, so the email script works well, but....... everytime Iactually submit it, the PDF brings up a box that says "Submit Cancelled" even though it goes through? Any Ideas how to fix that?

Not java savvy (unless it comes hot with foam) I have a submit button with mailto: address's. how do I get the CC to come from a field text box?

Im not sure how to make it conditional. but you could just make them visible if each box is checked and have them required too
