I'm not an expert, but I've notice on my forms with hides that sometimes when you open the file again after its been saved the show/hide form does appear to be hidden again.  However its still there.  If the radio button the user choose is selected, just select it again and the hidden form will show...

I believe its called Reader Extensions Server.  Someone else may know if that comes standard with PRO 9.0 Extended, I'm not sure.  Don't have any pricing info.

Its the question that we all ask at some point.  You have to enable reader rights using Adobe Professional (8 or above).  The problem with that is that you have a limit to the number of times the form can be submitted.  Adobe does have an add on, that eliminates that limit, but it costs.

Examples work best for me so that why I'm alway saving the sample files and template posted here and other places.  So here's an expense report that I believe does exactly what you are asking for.  Check the script underneath, I've copied part of it here, and I'm sure you'll be able to figure it out...

If I understand your question correctly you want repeating columns.  Someone posted a sample of how to do that previously, so I can't take credit for this script, but it works great.  See attached.
