Since you have applied AEM Service pack 9, then you must apply the corresponding 6.5.9 AEM Forms addon package only. Thanks!   

Can you please explain - "Background Fill color changes to red" of which object? of Radio button?Please check out this form, and let me know if this is what you are expecting. As you can see, the background color remains red when I select the textfield below. Thanks!

Have you applied the latest 6.5.11 and corresponding Forms add-on package? Also, please flush dispatcher cache and check the issue again after applying 6.5.11.

Add a rule like this:  /0002 { /type "allow" /method "POST" /url "/content/forms/af/generalformcs/generalform1/jcr:content/guideContainer/rootPanel/items/panel/items/summary.process.jsp" }      and restart the Dispatcher.
