Permissions to publish Adaptive Form?

We are trying to "publish/activate" an Adaptive Form.  The user is getting a permission denied.  

 

Can somebody tell me what roles are required to create, edit, and publish Adaptive forms?  I can't seem to find the correct documentation and many roles might be old.
