Can we create a custom destination in Adobe Experience Platform to send segments to?

Yes you can create a customer destination to send segments to.  

The destination SDK allows you to create your own connection to write to.

Another option is to use the custom personalization connection to write out data for personalized segments.

Can we use custom personalization to export to a custom destination as well?  What would be the best approach here as we would like to push segments to a third-party

if the requirement is to activate segments to a non-out of the box destination then using the Destination SDK to activate an audience is your best bet.  It also depends on the third party which you are attempting to activate to.  Most third party solutions will use a storage container to accept audience data for ingestion.
