facing issue in form rendering using form block in document based authoring in edge delivery service

I have followed all the steps mentioned in the official documentation for the rendering form, but I was unsuccessful.
Instead of showing the form the page shows me the link and I don't know what went wrong but it looks like I am not able to get the form rendered on the page.

I have used the template provided by Adobe for the tutorial:   aem-boilerplate-forms

please let me know what exactly am I doing wrong here,
