Pop up with few input fields required on double click on a table column

Hi Experts,

I am quite new to designing form & Scripting. I need to have a pop up window(with few input fields) whenever user double clicks on a table cell. Based on the value entered by user in the pop up, i need to change value in the table cell(on which user has double clicked.

That is pretty straight forward. The code is on the change event of the DDList. I created a sample for you ...see attached.
