Need help coding an options box that comes up when a PDF file is opened

I'm having the worst time trying to find answers to this...


What I have is a PDF document that consists of several individual forms.  I want to set up the whole document so that when you open it, it immediately creates an options box that allows you to choose the form(s) you want using check boxes, and when you click OK, it generates only the form(s) you select.  I know how to code the appearance of the options box, and I think the code goes in the docReady event, but beyond that I have no idea how to actually code it to work.

 

Even a link to a tutorial would be very helpful. Thanks in advance.

Before I can attempt to help you I need to understand your requirement better. How did you put in "several forms" into your Designer template? And how do you make them visibe/invisible? Do you have an example you can share with me in a private message?
