This should probably be in Forms ES forum but where are you trying to render the form from? Have you tried the sample applications, specifically the FormsIVS application to test the form rendering (just form and forms components). Sounds like a JNDI lookup error or is it only specific to your form. Probably would need to see the rest of the exception and it might be that you need to get adobe support if it is.
