Additional fields not saving

Hello

We have created a PDF document in Live cycle with the option of adding additional fields. After a certain amount of field lines have been added (in this instance its 15) you can no longer save additional information. Also when you open the document again, not only is the information gone but it has added a new blank line.

Hopefully Im making sense above but if not I am happy to email the document for viewing for a better understanding of what I mean. Can anyone assist in resolving this problem? Much appreciated
