Show pages if check box is true

Apologies if this have been asked before, I have searched the forums with no luck. I am setting up an application form which has some personal information on page 1 and then the option to select up to four check boxes. Each checkbox has an relavent application form. If the check box is true then I want the application form to be made visible but if its false I want the form to remain hidden. Do I need to save the application forms as seperate pdfs??



I have only been using the designer for a week and any help would be appreciated.
